MDMA affects one's body temperature regulating mechanism;
overheating is very possible, especially when actively dancing.
Drinking a reasonable amount of water is useful for cooling off,
both directly, and indirectly by sweating. Animal studies have
also shown neurotoxic effects are more likely at higher body
temperatures.
However, it is possible to drink too much water. One British young
woman, reading that drinking a lot of water is important for MDMA,
took some MDMA (but in a quiet setting, not dancing) and drank
so much water she died! So when taking MDMA, especially in a
quiet setting, a little water is OK but don't overdo it.

Nobody has died directly from the psychological effects of LSD.
Very rarely, someone might harm themselves or others; or, the
effects are so traumatic psychologically that someone predisposed
to schizophrenia could be pushed into a psychosis. The same can
happen to such a person from any psychological trauma (bad car
accident, death of one's parents or a lover, flunking out, etc.)
Schizophrenia typically starts to show itself in one's early 20's,
which is also the typical time to start experimenting with the
psychedelics.
